What it is. The Valsalva maneuver is a forced exhale against a closed airway — the same physical action as bearing down, straining, or blowing hard against a pinched nose. It was named after Antonio Maria Valsalva, the 18th-century Italian anatomist who described it. Doctors use it routinely in clinical settings to break supraventricular tachycardia (SVT), an abnormally fast heart rate, by triggering a strong vagal response. The technique works because the intrathoracic pressure change it creates directly stimulates the vagal nerve, producing an immediate parasympathetic shift.

How to do it.

The modified (supine) Valsalva, which produces the strongest vagal response, is used in clinical settings with patients lying down. For everyday anxiety regulation, the accessible version works as follows:

  1. Take a medium breath in — not maximum, about 75%
  2. Pinch your nose and close your mouth
  3. Exhale forcefully against the closed airway, as if trying to pop your ears or push the breath out through your nose — you should feel pressure build in your chest and head
  4. Hold the strain for 10–15 seconds
  5. Release and breathe normally

The parasympathetic rebound occurs on release, not during the strain itself. The heart rate may briefly increase during the strain (sympathetic activation) and then drops on release as the vagal response fires.

Simpler version. If the nose-pinch method feels awkward, any of the following produces the same intrathoracic pressure increase: - Blowing into a balloon or against a closed fist for 10–15 seconds - Sitting on the toilet and straining (the original context in which most people encounter this without knowing it) - Blowing hard through a small opening (pursed lips, a narrow straw)

When to use it. Acute anxiety, racing heart, the onset of a panic spiral. The Valsalva is not a subtle technique, but it produces one of the fastest heart rate reductions available without medication. It is particularly useful when the heart is pounding and other techniques (breathing, cold water) feel inaccessible or insufficiently strong.

Mechanism. Forced exhalation against resistance dramatically increases intrathoracic and intra-abdominal pressure. This compresses the vena cava, reducing blood return to the heart, which triggers baroreceptors in the aortic arch to signal the vagus nerve. The vagal efferent response slows the heart rate through the cardiac branch of the vagus nerve. On release, the pressure drops, blood rushes back to the heart, and a compensatory vagal rebound further slows the rate. The combined effect can reduce heart rate by 20–30 beats per minute within 30–60 seconds.

This is the same mechanism that causes people to faint when straining: if the vagal response is very strong and the person is standing, the heart rate and blood pressure drop fast enough to cause a brief loss of consciousness. Doing the technique seated reduces this risk.

Safety note. Avoid the Valsalva maneuver if you have a known heart condition, uncontrolled hypertension, glaucoma, or have recently had eye or ear surgery. The pressure increase is significant and is contraindicated in these contexts. If in doubt, use a less intense technique (physiological sigh, cold water to face) and consult a doctor before trying this one.
